If you have a lab kit and an order for blood work, we can draw your blood. If you have an order for blood work, but were told that we would provide the kit, please include those details below.

Any vials that require serum separation will be spun in our centrifuge, and then you will be responsible for packaging and mailing the lab kit.

After completing this short form, someone from our office will contact you to set up an appointment.
