Order Request

Our current hours are Monday - Friday, 9 am - 3 pm.

We are at our clinic in Moscow on Tuesdays, and provide mobile services in Idaho on other weekdays.

If you are wanting to schedule an appointment for a blood draw, please complete our lab draw form instead.

When you are finished, click Submit.

If you are a new customer, you’ll be asked to fill out a brief medical history. If you are a returning customer, you’ll be able to request an appointment on our calendar, with appointments often available within the hour.
